Sweet Pea Imaging provides both non-diagnostic elective ultrasound services for bonding and entertainment purposes and diagnostic ultrasound services read by a radiologist.

Elective:

Elective ultrasounds are not intended to replace or serve as a substitute for any medical examination, diagnosis, or care provided by a licensed healthcare provider. Clients should continue to receive regular prenatal care from a qualified obstetrician, midwife or healthcare professional. You grant Sweet Pea Imaging to use photographs, ultrasound images obtained during your exam or visit, for marketing and promotional purposes, including use on social media, our website, and other Sweet Pea Imaging marketing materials.

Fertility:

The services provided at Sweet Pea Imaging, for CNY Fertility patients; including vaginal ultrasounds and IVF monitoring, are for diagnostic and monitoring purposes only. These services do not guarantee results and do not replace medical care from any doctor / specialist. Procedures may cause mild discomfort or spotting. It is the duty of the patient to report any concerns to their provider. Information is kept private, unless given verbal / written consent by the patient. I release Sweet Pea Imaging and its staff from liability for complications, claims, damages, or liabilities arising from the procedures.

If you experience any medical symptoms, concerns, or questions regarding your pregnancy, please contact your healthcare provider immediately.

Diagnostic Ultrasound:

At Sweet Pea Imaging, we understand that your health information is personal. We are committed to protecting the privacy and security of your protected health information (PHI) in accordance with applicable federal and state laws, including the Health Insurance Portability and Accountability Act (HIPPA).

Signed Consent

By signing the consent form at checkout, you acknowledge that your signature constitutes your agreement to and acknowledgment of all applicable Sweet Pea Imaging Policies and terms, regardless of the type or purpose of your visit. This includes diagnostic, elective, fertility, pregnancy and any other services provided by Sweet Pea Imaging.

Your signature applies to all applicable policies associated with your visit and is not limited solely to the service(s) being provided or performed on the date of your appointment.
